Pre-Register & Prepare for Your Visit
To ensure your appointment runs smoothly, please contact your local campus to learn whether additional documents are required for your visit. In addition, please remember to bring current or past x-rays, CT scans or MRI scans, as well as imaging reports. If you do not bring these to your visit, we will have to reschedule you for a later date. We accept hard copies of these films as well as discs.
